Dependent Resident Card (TRC)
I prepared all the documents below (It took one month), and then sent an email to book a TRC slot in May. I got the slot after 3 months..
So, first booking a slot and then preparing the documents is better.
How to make an Appointment for Dependent TRC:
Send an e-mail containing the completed registration form to: kartapolaka@muw.pl
Email Subject: legalizacja pobytu – Applicant Full name

Agents will provide clarification in this helpdesk if you have any questions related to the TRC/Blue card.
