Subject: Inviting my son "NAME" on a Poland National Visa (Dependent)

Dear Sir/Madam,

I want to invite my son’s "NAME (Indian passport no: XXXXX)" to join me during my stay in Poland i.e. till the end of my visa date "starting from" his "travel date (i.e. xxx days)".

I, undersigned, (son’s NAME) citizen of India, with passport number (SON PASSPORT NUMBER), hereby declare, that I am employed by (COMPANY NAME) based on an unlimited time Polish employment contract & work permit (typ. A nr. 14) Issued by Malopolskie voivodeship office. I have a monthly salary of (AMOUNT)PLN gross.

Once his visa is issued I’ll travel to India, and I’ll accompany my son (son’s NAME) and my wife (WIFE NAME) on their travel from India to Poland. I also declare that, due to my absence in India at the moment (for my employment in Poland), I also authorize my wife (WIFE NAME) to sign and carry out the visa application process for my son (son’s NAME) on our behalf.

My employer has made all the arrangements for our travel reservations.

I have rented an appropriate accommodation for three of us i.e. a flat with 2 rooms, lobby, kitchen and washroom located at (FLAT ADDRESS), Poland.

Simultaneously, I declare that I will take responsibility for my family’s stay on the territory of Poland, and in accordance with Polish law, I will cover ALL his living expenses, e.g., Medical insurance, accommodation, food, studies, and any other costs, together with the cost of returning to the country of origin. I hold a Polish Bank account with (BANK NAME) with a balance of (AMOUNT) PLN (approx.) at the moment (which I suppose is enough considering 75PLN/day for 143 days + 2500 PLN). Of course my regular salary would support my financial needs even better. I have attached the balance statement from the bank for your reference.

I’ll purchase the travel medical insurance for my son with coverage of a minimum of $50,000 which covers the whole duration of his stay in Poland.

Another supporting invitation letter from my company is also enclosed with this letter.

I kindly request you to grant the multiple entry national visa (dependent) for my son to accompany me during my stay in Poland. Please do not hesitate to contact me for any queries.
