How to apply for the power of attorney in the Indian Embassy
Step one: send email to cons.warsaw@mea.gov.in

I would like to request an appointment to get an Attestation of signature in a Power of Attorney. Can you please let me know the process for the same?

Step 2: Visit the embassy with the below document
The documents required for Attestation of the Power of Attorney:
- Miscellaneous form duly filled in and signed (the form is available here https://www.indianembassywarsaw.gov.in/images//Misc_Services_form.pdf ),
- Original valid passport and Polish/Lithuanian Resident Card/Visa,
- Self-attested photocopy of the first and last pages of the valid passport,
- One recent photograph,
- Concerned document in duplicate,
Please note you have to visit the Embassy with the required photocopies, application form etc. Details can be found at the Embassy's website.
Consular fees of 107 PLN are applicable for the Attestation of one document.
Fees are to be paid by card while submitting the application.
The document will be ready for the collection on the same day from 4 pm. to 5 pm.
